As a plan participant, you have your own personalized Total Access FSA web portal where
you can learn and check account activity at any time throughout the year.

Through your web portal, you can:

> Learn about your Total Access FSA program
> Enroll online during open enrollment
> Confirm your participation in the plan
> Check your annual target amount, contributions deducted year-to-date and available plan balances
> View processed claims and reimbursement amounts
> File a claim online (Transit and Parking FSAs only)

How do I log on to my Total Access FSA web portal?
To access your online account, go to the web address assigned by your employer and use your individual User ID and password.

What's the web address?
The web address for the Total Access FSA web portal starts with your company name in place
of the customary "www" and ends with ".mybenergy.com." So, it will look like this: http://yourcompanyname.mybenergy.com. For questions, send an e-mail to fsa@benergyos.com
or contact us by phone at 800-768-4909. Be sure to identify yourself and your employer.

What's my User ID?
Your User ID was assigned to you when you enrolled in Total Access FSA. It is your Social Security Number separated with hyphens. So, it will look like this: 123-45-6789. For questions,
send an e-mail to fsa@benergyos.com or contact us by phone at 800-768-4909. Be sure to
identify yourself and your employer.

What's my password?
Your password was assigned to you when you enrolled in Total Access FSA. If you are accessing the portal for the first time, you will need to change your password and answer a security question.

If you forgot your password, you can retrieve it from the portal by answering your security question. Once entered, your password will be e-mailed to you. If you have questions, send an e-mail to fsa@benergyos.com or contact us by phone at 800-768-4909. Be sure to identify yourself and
your employer.

How do I file a claim?
You can file claims in several ways:
> By paper form. You can find the appropriate claim in your Total Access FSA web portal.
Fill out a claim form and mail or fax to Benergy OS.
> Online (for Transit and Parking FSAs only). Go to the Total Access FSA web portal and look in "My Info" for details. (Be sure to scan receipts and send with your e-mail.)
> By flex card. If you received a Total Access FSA Flex Card, you simply use the card at the time you pay for your eligible expenses just as you would any credit card.

How do I receive reimbursement?
You choose how to receive money:
> By check. Simply mail or fax your claim form to Benergy OS and you will receive a check in the mail.
> By direct deposit. If available, you may have your reimbursements placed directly into your bank account. Simply indicate your bank account number and routing number on the Total Access FSA Direct Deposit Form. (The form can be completed at any time.)
> By flex card. If you are given a Total Access FSA Flex Card, you are reimbursed immediately from your account and do not even need to file for reimbursement.
